Work Permits

Please email mohammad.rafiq@sanjuan.edu to make an appointment for your work permit. 

Work permit applications may be picked up in classroom 25, or if that door is locked, in the Principal's Office.  Once you have a job, you will have your employer fill out the form and sign it; have a parent/guardian sign the form and then return it to Mr. Rafiq in classroom 25.
